Mother Holly will appear 666 meters away from the player. She will slowly approach, getting larger and more visible as she gets closer.
Staring at Mother Holly will cause her to retreat. If Mother Holly gets too close, she will instantly kill the player.

The main way Mother Holly kills is by hoping you forget about her or otherwise getting distracted by other terrors. Always keep her in the back of your mind, and check on her every 20 seconds or so to make sure she's not too close. This is difficult due to the lack of audio cues (until it's too late), so do whatever you can to remind yourself that she's there.

VR mode still retains a sort of reticle that Mother Holly uses, so if you can memorize where, you'll more likely be fine
